WHAT IS PVI: PREGNANCY VIABILITY INDEX

According to one study it is possible to predict which pregnancies may make it and which ones will not.  There are six factors which can help predict whether or not a pregnancy will be successful:
  •  a history of subfertility
  • levels of progesterone
  • levels of hCG
  • the length of the fetus
  • how much bleeding had occurred
  • gestational age of the baby

However two of these factors. the amount of bleeding and levels of hCG were combined to create the "Pregnancy Viability Index" (PVI).   PVI was able to accurately predict the pregnancy outcome in 94% of women who had ongoing pregnancies.

CAN FISH OILS HELP PREVENT MISCARRIAGE?


I frequently write about the benefits of omega 3 fatty acids.  They are crucial for hormone development and aside from all of the benefits of omega-3’s, there may be evidence that they can help prevent miscarriage.  
Pregnant women who take fish oil supplements may cut miscarriage risk due to inflammation of the placenta, and also improve the function of placenta.
 
As published in the Journal of Lipid Research, omega 3 fatty acids (fish Oil) affected the placenta and fetus of pregnant laboratory animals. After fish oil supplementation, the placenta had higher levels of compounds called resolvins. The study concluded that the omega-3 fatty acids present in fish oil can help limit inflammation in the placenta.[1]

PREGNANCY AFTER RECURRENT MISCARRIAGE, ENCOURAGING STATS

Even though women who experience recurrent miscarriage must endure the pain and heartache of losing pregnancy after pregnancy, the fact of the matter is that most of these women go on to have a live birth.

The researchers now intend to follow up their work by looking at the time it took to achieve a live birth as opposed to simply time to pregnancy. "This will be important in counselling these women for their future pregnancy attempts," said Dr. Kaandorp. "We hope that our work will encourage health professionals to avoid non evidenced-based and potentially harmful treatments for couples. For example, aspirin and low-molecular-weight heparin are still being used, even though the ALIFE study showed clearly that there is no evidence that they are helpful.
"Our results mean that women with RM can be reassured that their time to a subsequent conception is not significantly longer than that for fertile women without a history of miscarriage. RM is extremely stressful for these women and we hope that our study will give them hope and encourage them to keep trying for the baby they want so much," he concluded...

... The cumulative incidence of natural conception was 56% after six months, 74% after 12 months, and 86% after 24 months of which 65% resulted in a live birth.

MISCARRIAGE, SAYING HELLO BEFORE SAYING GOODBYE

So many women experience miscarriage but feel lost on how to deal with it. Sometimes others either don't know about the pregnancy, or if they do know, they say the wrong thing. I do recall some pretty insensitive comments (especially by healthcare professionals who should probably know better.) Saying things like, "You can always try again" or "Have you considered adoption?" felt like a knife cutting in my side. I just wanted to scream at these people...it felt like they just wanted to sweep my lost pregnancy under the rug and pretend it didn't happen. This article has some words of wisdom on how to deal with miscarriage:

Disappointment didn't begin to explain my feelings. This was the second miscarriage in six months. Having one child already, in my heart I wanted so much for him to have a sibling. I was grateful to my doctor for understanding my devastation and for my husband's attempt of support but there were very few others who understood the depth of feelings I was experiencing. This happened in the early seventies.
During the 1980's I became involved in the beginnings of the hospice movement. In my training I learned much about grief mostly unknown to the general public. The most important lesson I learned was to help grieving families personalize their celebration of their loved one. After five years at hospice, I went to work as a bereavement director at a nearby funeral home. I received an education unlike any other. The funeral director highly encouraged direct participation in the funeral ritual to promote physical and emotional healing.
One young couple whose first child was miscarried came to us for help. We encouraged them to name their baby and tell us all the wonderful things that happened to them when they found out they were pregnant. Mommy and Daddy shared many special memories with us. In the midst of their sorrow, they smiled through their tears sharing the hopes and dreams they had in the first few weeks of the pregnancy. How disappointed they were when their dreams ended in miscarriage. We helped them make arrangements for a simple memorial service.
Their little one was tiny, only about eight inches long weighing less than two pounds. I found a tiny doll outfit that the baby could wear. We arranged a viewing in a small intimate room in the funeral home as if it were a friendly bedroom. I placed an afghan on a rocking chair next to the tiny casket, an angel night light, a few stuffed animals and a child's board book. The appointed time came for the parents to see their little one--to first say hello and then goodbye.
Mom and Dad were visibly moved the moment they walked in the door. There were tears but also smiles. We left them alone with a few encouraging words. After some time, they indicated they were ready for the next step. Their parents and loved ones drifted in a few at a time to lend their support. The healing had begun.

People, who have had bad experiences with death or no experience at all grieving over the death of someone they love, often think something like this scenario sounds macabre but those who have know that something special just took place and they now have some beautiful memories of their child.
Years later a grandchild of mine was miscarried. I had the privilege of making a little blanket for him and purchasing a tiny toy that was tucked into the small wooden box my son and his wife purchased and decorated. His daddy conducted his funeral service. Family and friends prayed and sang together as we comforted mom and dad and each other. The healing had begun.
So, what can be done if a friend has a miscarriage? First contact them with words of love, caring and your own disappointment. Any positive thoughts about your feelings when you heard about the pregnancy would be greatly appreciated for they are a tangible proof that this child touched your life. Perhaps you could find a special piece of jewelry, a window prism or other small memento to give the parents as a gift in memory and celebration of their child. Another token of your caring could be a perennial flowering plant or tree. Anything that will reinforce that this tiny human being made a mark on the world will give hope and comfort to his or her parents. I wish I would have had this knowledge when my own miscarriage occurred. I'm sure I would have healed much faster.
Knowing what I now know, I would encourage moms who've miscarried to write about their feelings and observations in a journal set aside especially for this baby. Making something like a stepping stone, a small scrapbook or planting a specially chosen perennial will bring some closure. A cement lamb or duck set in a special place in your yard is another idea couples have found helpful. The most important thing is to do something in honor of your child even if it's to take a long walk. Every child makes an impact in the world no matter how little. Begin the healing.

SHOULD YOU HAVE A D&C AFTER MISCARRIAGE?

D&C stands for dilatation and curettage and it is sometimes recommended after a miscarriage which has not expelled (or has not completely expelled) after a pregnancy is no longer viable.  Most of the time, after a fetus has died, a woman will spontaneously bleed and pass the remaining tissue.  However, this can take some time because the pregnancy hormone can stay in your system quite a while.  When the pregnancy hormone is still in your system, your body can still "think" it's pregnant and the lining of your uterus stays in place.

WHAT CAUSES MISCARRIGE BY ECTOPIC?

Ectopic pregnancies are particularly devastating if you're trying to conceive. More often than not, you will need to undergo surgery to remove the pregnancy because if it ruptures, you could potentially bleed to death. For many women, this will require the removal of one of their fallopian tubes (if the pregnancy lodged there - although some ectopic pregnancies are in the abdomen).

WHAT IS THREATENED MISCARRIAGE?

MEN'S DRINKING CAN LEAD TO WOMAN'S MISCARRIAGE

We all know women aren't supposed to drink alcohol if they're trying to conceive or if they're pregnant.
 Now, it seems that the same holds true for men.
I found it astounding that if a woman had a male partner who consumed ten or more drinks per week, she was five times more likely to miscarry. So it's not just women who can help lessen the likelihood of miscarriage.

HOW DO YOU KNOW IF YOU'RE HAVING A MISCARRIAGE?

Loss of Pregnancy Symptoms and Other Ways To Know If You're Having a Miscarriage

The thought that something might be going wrong with your pregnancy is mortifying. If you've never had a miscarriage, you may not know what to look for.
 I had spotting with all my pregnancies, even my successful one, so spotting alone doesn't mean that you are going to lose the pregnancy.  Usually the spotting was "old" blood, so it was dark, but when the spotting turned brighter red or got heavier, I knew I was probably going to lose the pregnancy.  In addition, when I lost my pregnancy symptoms, especially the fairly severe nausea, I knew there was a problem.

1.Pregnancy symptoms can fluctuate even in a normal pregnancy. Remember that loss of pregnancy symptoms does not equal a miscarriage, although you may want to mention this to your doctor.

2.In early pregnancy, do not try to figure out whether you are miscarrying by taking home pregnancy tests to see if the line gets darker. Home pregnancy tests cannot accurately judge how your hCG levels are rising and the darkness of the line can vary based on the time of day and amount of water you've been drinking.

3.If you are Rh negative and believe you are having a miscarriage, remember that you may need a RhoGAM shot. Be sure to mention this to your doctor.

4.Note that this list assumes you are concerned about first-trimester miscarriage symptoms; if you are in a later stage of pregnancy and worried about miscarriage, your first step should always be to call your doctor.

MISCARRIAGE AND ENDOMETRIOSIS, CONNECTED

It seems most experts agree that endometriosis can be a definite cause of infertility, but once you get pregnant, can it cause or contribute to miscarriage? Half of women with endometriosis will have problems with infertility. Many of these women, up to fifty percent, also miscarry when they do finally become pregnant. In comparison, the rate of spontaneous miscarriages for the general population is about ten percent. So why do women with endometriosis have so much more trouble becoming and staying pregnant? There are several factors related to endometriosis that make it difficult for women with this condition to conceive and carry to term.
Abdominal Adhesions

The blood and tissue leaked by endometrial tissue into the abdomen can be harmful. In an attempt to protect itself, the body surrounds these patches with fibrous scar tissue. This scar tissue, combined with the blood from the implanted endometrial tissue, can cause these patches to stick to other organs in the vicinity.

Depending on which organs become glued together, effects may vary. If the fallopian tubes become stuck to the ovaries, or if the fallopian tube is pinched off in some way, this will prevent the egg from traveling from the ovary down through the fallopian tube. This, of course, results in infertility.

Depending on which organs are connected and how, these abdominal adhesions can also result in miscarriages. Ectopic pregnancies, pregnancies outside the uterus, are also more common in women with abdominal adhesions.
Implant Secretions

The endometrial tissue secretes many important hormones that help facilitate pregnancy. The endometrial implants secret these same hormones, but instead of secreting them into the uterus where they belong, they’ll secrete them into the abdominal cavity or other parts of the body. These misplaced hormones may interfere with the pregnancy process.

In addition, the implants often secrete hormones later than the cells lining the womb. This may cause the body to become confused, and continue ovulating even though a fertilized egg has already been implanted. When this occurs, the uterus sheds the old lining, including the egg, and a very early miscarriage results.
The Immune System

It is possible that the immune system may also play a role in the high infertility and miscarriage rates in women with endometriosis. The developing embryo is a foreign organism. A healthy immune system can identify it as not being a threat, but since the immune system of women with endometriosis is not functioning properly, it may not be able to make this distinction, and might attack the embryo, resulting in miscarriage.

57 YEAR OLD THOUGHT TO HAVE MISCARRIAGE WAS PREGNANT

Very strange things can happen in our reproductive systems.  Here is a story about a 57 year old woman who underwent IVF, and thought she had miscarried.  Fast forward months later, an "abdominal mass" is thought to be ovarian cancer.

 It is later found that the woman is indeed pregnant from her IVF procedure.  She went from having a death sentence to a having the joy of a new life.   Unfortunately, many older women have a misdiagnosis when it comes to pregnancy because doctors thing they're too old.

Not only is she about to become a first-time mother at the age of 57, but she found out she was pregnant just eight weeks ago.
Referred to hospital as an emergency after a physical examination by a private GP had revealed a "hard abdominal mass", she had been bracing herself for the worst.
Clutching a paper with the suspected diagnosis "Ovarian Cancer?????" on it, she burst into tears as she waited for a scan.
"I was lying on the examination table thinking, 'I'm going to die' when the sonographer turned round to me and said 'Congratulations'," says Susan.
"My initial reaction was to think, 'What a terrible way to tell me I've got ovarian cancer'. When he then said, 'You're pregnant' I was literally speechless.
"I still couldn't believe it when he showed me the screen and said, 'Here's the head, here are the arms and here are the legs'. I was even more shocked when they told me I was almost 30 weeks gone."
